New fields in course browser/map browser

Just added conditions and year established to the course browser and the map browser. Seems to work so far but if you run into any trouble please let me know. If it doesn't work on the map for you, try a Ctrl-F5 as you might have to reload the javascript file.

Both of the forms got reworked a little as well to squeeze in the new fields.

You can now filter courses by those fields.
