Looks like you are almost pulling your arm through at your waist. Try bringing your arm up way higher. Chest/shoulder level. This should help. Almost impossible to keep the disc on one plane when you throw with your arm that low. Looks like you are a little behind your throw also. Like not leaning forward enough.
