Repetition in particularly similar situations. Make practice more like the round and change they way you think during rounds. Minimize the motions that will cause variations. Make sure you practice the same form over and over until your body will simply do the same thing over and over again. Practice in the same way that you play.

Agree!! Be agressive!! Give the disc a chance to go in. Putt through the chains so they have to stop the disc. Not into the basket! Follow through with your arm. You might have to raise your arm much higher than you think. The basket might disappear. Where your arm goes is where the disc goes so get it right at the basket.
