Getting basket sponsers

So I am trying to get sponsors locally for my home course's back 9 baskets for this summer Alcantra DGC in Alaska But I have come into a snag as to where I cant really decide how to make a form for businesses/individuals to fill out in order to sponsor a hole and I was hoping some of you guys that had already done this might have a base pdf/txt document already made up that I could edit and use as our own to fit the bill.

i can do the gfx np. did it for years and do it for local rc events for local clubs
