Golf/Disc Golf Courses - Help Needed

A local parks department is exploring alternative plans for an existing nine hole golf course. One option being considered is adding disc golf to the existing golf course. I have been asked to provide a list of golf/disc golf courses. I check DGCR's course search and do not see that option so I am here asking for your help.

Please post the names of any golf/disc golf courses. Ideally, include the DGCR course listing link.

I sent a similar list to a guy that owns a ball golf course recently that was considering adding disc golf. If mashnut's list and the others don't suffice let me know and I can recreate the list when I get a chance. There aren't a ton of them, maybe 10 or 15 more than what is already in this thread.
