You are turning back too far and then moving forward. You need to stride forward while you are turning back.

Your reachback is also really high. Play around with the Door Frame Drills and a lower reachback height to leverage your body into.

Your rear elbow is flying out away from you. Related to turning back too far, where your elbow has to go back away from target when you start swinging forward. Make your rear elbow move more linearly back to front into the plant into your hip, rather than the rear arm going around rotationally. https://www.youtube.com/watch?v=Mpp7ZFLHK90&t=9m40s
