Need som help with my form

I have been stuck at a max distance of about 120 m (400 ft) and don't know what the next step is to improve. My very ambitious goal is to throw over 550 ft and get a 1000+ rating in the future but I would need some help. Any advice relating to distance gainz would be helpful
