Hey! I would love to help. I think the best thing I can offer is the use of a quality slow motion camera and maybe a few tips here and there. Looks like you've already got some solid form. Getting a couple different views of your throw in slow motion can help you diagnose what you feel like your doing vs what you're actually doing. Hips turning/shoulders turned, getting that reach back out from under your off arm, whether the disc is slipping out or ripping out etc. Its also a great resource for posting on these or other forums for even more feedback. I've also got a big ass measuring tape which helps put a solid number down to measure your results.
Putting in some solid practice (field work, getting out of your comfort zone during practice rounds by working on what you feel to be your weakest aspect) will get you the best results, but it definitely takes some effort.
