I have a puddle top esp zone that has been in the bag for 4 years, throw it as hard as I want, straight to 250-300, then fade, great on a forehand too. The most consistent disc in the bag.

It actually sounds like a pinched nerve. When I first started throwing forehand the same thing happened to me, I was not smooth and trying to power everything. Ice, ibuprofen and rest helped. I had a buddy that had the same symptoms, required surgery to move the nerve, his was from pitching.
Duct tape and ribbon saved me a few discs in KC when I played snow golf. Works great, even if it sinks the ribbon sticks out, doesn't affect the flight much if any.
