Enter Madison on State Highway 34. At the intersection of North Washington Avenue and NE Second Street, go east (on NE 2nd St, which becomes NE 3rd St) for about 3/4 mile. The park is on the south side of the road. The first tee is next to the entrance to gravel parking lot.
