You are very flat footed, your rear hip is trailing the throw as your rear leg kicks up and around in the finish and you are rounding your pull line. Get athletic on toes more, close your stance, maintain your posture and balance, don't push off your rear heel, and swing straight through your core.

 
You turn your back to the target and reach back WAY too early.

You are completely flat footed. Being up on the toes will get you more athletic and you'll be able to generate power with your lower body.

You lean back over your back foot when you should be staying upright and turning back. Turn back; don't lean back.

You come off of your back heel rather than your back toes, and your back foot is pointing too far away from the target. It should be closer to 90 degrees.

There is no bracing of your weight against the front foot, and your weight should shift to your front foot BEFORE you throw.
 
Looking better with the balance and stance, but still pushing off your rear heel. Guessing you are also throwing some really fast and or overstable discs throwing an anhyzer line like that. I'd suggest discing down to putters/mids/fairway drivers and learning to throw flat/level or hyzer.
