Next steps on my form?

My form was bad and I've been working hard on it for about a year now. I've got it to be much better now however I'm not sure how to level up from here.

I used to throw 300' with bad form and now I throw 300' with much better form.
But my accuracy is better so at least there's that.
I can hit 330ish every few throws, and I've had a few sail out to 360 by chance.

I'm putting in the field work often, and and lot's of it.

Please have a look at my vid and give me some feedback as to what might be good for me to focus on going forward.

1. Need to shift forward closed. Note how your front leg spins open going into the plant before your weight shifts.

2. Need to grip the opposite side of the disc to leverage it. Note how you are gripping toward the front side of the disc and swinging very downward with disc nose up.

Update: Still working on it... in progress. A work in prog.. yeah.
Actually putting a good amount of time into it.

Thanks again SW22. You're feedback has been most helpful and I appreciate what you do here. Additionally, I had a lesson/session with a local pro (Ziggy Bierekoven) last week and that was icing on the cake. I've been working my ass off in the fields and on the green and am exited to be seeing improvements. My average distances and accuracy is getting much better. I was able to hammer my best drive on this hole a few days ago. Feeling good.
