Form help for distance

Thanks in advance for the help. Just some background, I'm 6'4 207LB. I'm in good shape and I play at least once a week. I've been playing for 12 years. I should be able to throw much farther than I do. It has to be mechanical/timing.

I used to be able to throw 420 max and sat around 380. For the last 7 years, I have not been able to even break 400. Problems I typically have:

- Low release (throw into ground)
- Timing
- Disc fades out quickly
- Disc seems to just die
- Can't get the disc high enough
- Bad brace (I've had knee surgery on both knees so I'm worried about hyperextending my knee too far and hurting it again.)

The first link is a Destiny. Here is what I see I'm doing wrong:
-dropping my arm down to hyzer way too much
-bad brace
-slow arm
-bad timing (reach back at max before front foot comes down

The second link is a week later with the aforementioned observations in mind. Disc is an Enigma. Here's what I see I'm doing wrong in this one:
-bad brace
-bad timing (reach back at max before front foot comes down)
-slow arm

Obviously, I have a lot wrong here. I think I know what I'm doing wrong, I just can't get my body to do what my brain is telling it.

I would like to know two things please:

1.) What am I doing wrong? Everything please?
2.) How can I train myself to fix each wrong thing?
