If you can't tell what is going on, the disc got wedged in the S hook and was stuck like that. Actually damaged the disc in the process and it was a bitch to get out.

Definitely in, supported entirely by the chains so it counts. I've never seen a disc do that, I guess it would have to be a driver moving pretty quick to be able to wedge in like that.
